Instructions for Visa Interview
Please bring the following documents with you to your visa interview. All documents must be originals or certified copies.
- Two photographs (5X5 cm), only if not already submitted
- Photocopy of the biographic pages of your current and prior passports pages and pages that contain visas and stamps (current passport must be valid)
- Police certificate (s) from any country (ies) in which you have resided for more than six months after the age of 16 + translation (these must have been obtained no more than six months prior to your interview date)
- Form DS-230 (PDF 100K), parts I and II (all questions on the form must be answered)
- Family Extract with English translation
- Individual Civil record with English translation
- Birth certificate with English translation for children under the age of 14
- Marriage certificate with English translation
- Death certificate, if applicable, with English translation
- Divorce certificate(s), if applicable, with English translation
- Medical Examination Report (to be completed by a panel physician, please click here for instructions).
- An original, signed Affidavit of Support (I-864) from the petitioner and joint sponsor (if necessary). The supporting documents below are also required
- Copies of the petitioner’s/joint sponsor’s complete federal tax returns (form IRS-1040 + Schedules + W2) for the past year, along with a proof of Citizenship. These items are also required for the I-864, if applicable:
* Proof of the petitioner’s/joint sponsor’s U.S. domicile
* Proof of the petitioner’s/joint sponsor’s assets
