Passport Information

Applying for a U.S. Passport

As of April 3, 2008, American citizens requiring passport renewals, additional passport pages, and replacement for lost or stolen passports may visit the American Citizen Services Section on Mondays, Tuesdays, or Thursdays between 8:00 and 11:00 a.m.

Applications by U.S. citizen minors under the age of 16

  1. Child must be present.
  2. Child's official, original birth certificate.
  3. Previous U.S. passport.
  4. One matte photo (see photo requirements).
  5. Both parents must be present with identification, preferably in English. Names of parents on the identification should match their names as shown on the child's birth certificate.
  6. If one parent is residing outside of Lebanon, that parent must complete and notarize Form DS 3053 in the presence of an American Notary Public or American Consular Officer giving permission to the other parent to renew the child's passport.
  7. In case of death of one parent, the custodial parent should submit a Death Certificate.
  8. In case of divorce, both parents' signature or DS-3053 must still be presented unless one parent has sole custody. A divorce decree with custody authority granted by an appropriate court is required as proof of sole custody, with translation into English by a sworn translator.
  9. Photos of the progressive aging and physical changes of the child if the photo shown on the old passport does not reflect current physical appearance.
  10. Complete Form DS-11.
  11. Fee of US $85 or the equivalent in Lebanese currency.

Fees

The equivalent in Lebanese currency is acceptable for any of these fees.

Reporting and Replacing a lost or Stolen Passport

The following must be submitted by the applicant in person:

Police report, translated into English by a sworn translator.

One matte photo (see photo requirements).

Proof of US citizenship, if available, such as a U.S. birth certificate, Report of Birth Abroad, or Naturalization Certificate.

Evidence to establish identity, such as a Driver's License, ID card, Lebanese or other passport.

Completed Form DSP-64 and Form DS-11.

Fee of US $100, or the equivalent in Lebanese currency.

Extra Passport Pages

Form DS-4085 should be submitted along with the passport. The passport with the added pages will be given back the next business day. There is no fee for this service.
